Professional Referrals

To get started, we request a referral be sent to us by the current treating professional or facility.

Information needed to make a referral includes:

  • Current psychiatric evaluation
  • History and physical records
  • Height and weight information
  • Psychological/neuropsychological testing results
  • Any medical concerns
  • Past treatment history
  • IQ or approximate functioning level
